IP and FQDN cache cleanup

Looking at those caches contents, it seems that entries' TTLs are
only cleaned up when an access is attempted, or when the cleanup algo
gets close to the high watermark.

Since the cleanup algo has to sweep the cache anyways, whouldn't it be
better if it
just removed expired entries also?
