Everything MISS_DIRECT

OK,
I obviously have stuffed the configuration somewhere, Squid appears to be
loading the files into the cache but never retrieves them from it. Nearly
everythin in the access.log says MISS_DIRECT. Where do I start looking?

I have not changed the default settings for things such as never direct and
always_direct.
