Depends on the configuration/setup. The most common one for transparent
caches is without NAT, while the most common one for load balancing of
TCP services is with NAT.

When not using NAT the proxy host must be configured to transparently
accept traffic redirected to it. See the Squid FAQ entry on transparent
caching.
